CRM Notes Widget Cancel The title field is required!

   I've added the Notes widget to a form but I'm not able to get the Add button to work or the list of existing notes to display from the related entity.  I'm working with the Case entity and am able to pull data into the form and submit it back.  Based on the documentation it says to just pull the widget onto the form and that's it.  I've even tried pulling in an additional TPC form widget and associated it with Note and tried placing the Notes widget within the nested form control but no luck.  I am able to upload attachments thought on the Add version of the form.  I am on the most current version of TPC and Sitefinity 14.1.
