   Hello Krishna, 
   What version of TPC are you on? Also how do you have your form set up for the user to input a title and description of a note, this is a standard widget that has not had any changes done to it for a very long time. The easiest way for us to help you with this issue would be to get a look at your environment.    I got into a call and from that I am understanding that its not possible to set description with the file upload widget when a new record is getting created

   Hello Krishna,
   Unfortunately as discussed on our call the Title and Description of the file upload widget cant be changed dynamically in the current version of TPC. This might be a functionality that we add in the future, but at the moment it is not a functionality that is available.
